Formularize is an advanced form building application, That can help you build powerful data drive forms like surveys or quizzes. With its easy to use drag and drop form builder, makes it easy to build a form and maintain it. Collect data from the forms that can be displayed as aggregate data in charts and "testimonial" type blocks with anonymity. Check out below the features of formularize.

  • Future Publishing: keeps a form in a hidden state till date is reached.
  • Form Expiration: have form expire and auto lock on a certain date.
  • Max Submissions: sometimes you only need a certain amount of responses to a form, you can limit how many times the form can be taken in total before it auto locks.
  • Multiple Submissions: control if the forms can be taken a single time or multiple times.
  • Min. Form Completion: you might not want to require every form field, but you want the form to have x amount of percentage to be field out before it is allowed to be submitted, control how much of the form MUST be completed before it is submitted.
  • Conditionals: control the flow of a form, show or hide questions based on responses from other questions. note: not all fields can be used as conditionals, but a good majority can be. 
  • Privacy Notification: set a privacy policy in the ACP for the forms, and forms will require an acceptance of this privacy policy before taking the form. 
  • Demographic Field: a set of fields that can be used to collect the most common of demographic data.
  • Widgets: Dismissable Form list, Form List, High Scores, along with a 4th "global widget" that is similar to the dismissable form list, but shows up any page as a modal popup, which can be dismissed for x amount of days. will show a fresh list of forms you might want users to participate in.
  • Theme Settings: most colors used in formularize that aren't shared with other IPS theme settings, all have their own theme settings for easy styling to match your theme.
  • Notifications: standard notifications for new Forms on folders and new submissions for forms. along with moderator notifications for reporting a form or a form awaiting approval (along with submissions). 
  • Form Field Tracking: track interactions/impressions/time for each form field individually or averages on all the submissions.

  • Integration with stratagem: adds custom fields to stratagem projects/cards when formularize is installed with stratagem and post cards to existing or create new projects, for members to track their progress on various things, like use in learning modules.

Bug Fixes:

  • fixed notifications: was double sending notifications for form creation and was also sending notifications to form author (due to the the author return \IPS\formularize\Member instead of \IPS\Member and IPS code is comparing the objects, not the member_id)
  • fixed "last poster" information for folders and forms, was showing the incorrect time and user most of times.
  • fixed missing lang string for one of the widgets
  • fixed High Score side block and widget, as it wasn't showing scores (this was due to the scores not being recorded).
  • fixed steps and required fields, was halting the step progression if the field was required, even if the field was filled in. 
  • fixed quiz timer, was not submitting the form at expiration.

New Features

not quite "new" features, but more of a refinement to existing features.

  • published config option remove, replaced with "activation/deactivation" on form builder. this was due to a change the form config, to make form activation more readily accessible.
  • form creation, moved to a dialog window instead of a "hidden section" on the form builder, as the form wasn't working correctly (fields that should've been hidden were showing, cause of a race condition on the JS).
  • add tabs for form creation/config, to make the form easier to navigate and understand what each field option does.
  • can now switch between standard and quiz, and wizard and quiz wizard after form creation. i originally locked the form type after creation cause of the steps and tabs division between the standard (tabs) and wizard (steps) type of forms, as the tabs and steps have different requirements. wizards don't support tabs and standard doesn't support steps.

I LOVE this tool! But I'm getting an error that I can't figure out. Hoping you can help me. [[Template formularize/front/submissions/rows is throwing an error. This theme may be out of date. Run the support tool in the AdminCP to restore the default theme.]]

Please let me know if there's something I can do. I've ran the support tool and see no issues but I'm sure I'm missing something. 

Would also love a way to export the results to a spreadsheet for further analyzing.

version 1.1.0 will have export to csv for form submissions. there will be options to what fields to include (all the field items for the form will be included, but you will be able to include additional information like member id, member name, email address, ip address, date, updated date, stats (impressions/interactions/time) and if it is a quiz you will be to select the score, how many correct, total score of the quiz). the options to include like email and ip address will only present to groups who can see those based on permissions for their group/moderator status. 

You contacted me about the theme issue, and said its no longer happening, if there are continuing issues please let me know.
